Abstract:
Palatopharyngoplasty is a cornerstone of obstructive sleep apnea (OSA) treatment, yet clinical outcomes vary significantly. This presentation utilizes drug-induced sleep endoscopy (DISE) findings and intraoperative anatomical variations to investigate why these surgical outcomes differ among patients.
On the efficacy side, well-selected candidates experience substantial relief, characterized by a significant drop in AHI, improved oxygen saturation, and enhanced daytime alertness due to reliable retropalatal airway expansion. However, the surgery faces critical limitations. The primary barrier to success is unaddressed multilevel obstruction, particularly concomitant tongue base collapse or severe hypopharyngeal narrowing. Furthermore, DISE reveals that lateral or concentric collapse patterns at the palate level respond poorly compared to simple anteroposterior collapse.
In conclusion, palatopharyngoplasty is not a one-size-fits-all solution. Precise patient selection and comprehensive airway evaluation are paramount to avoiding surgical failures and optimizing long-term therapeutic outcomes.
