Abstract:
Sleep is a vital, active, restorative process for the body and mind. The physiological sleep-wake cycle relies on the interaction between homeostatic sleep pressure accumulating during wake (Process S) and the 24-hour circadian clock alerting signal (Process C). As night falls, the pineal gland releases melatonin, the biological signal of darkness, which has two main effects: it dampens the circadian clock's alerting signal to promote sleep and adjusts the clock phase to align the 24-hour sleep-wake cycle, with the day-night cycle. Chronic circadian disruption degrades sleep quality, which correlates directly with long-term cardiovascular, metabolic, cognitive, and mental health risks.
Insomnia is dissatisfaction with the amount and/or quality of sleep that is associated with significant daytime distress. Managing insomnia in adults aged 55 and older requires solutions that avoid traditional sedative risks like memory loss, falls, dependence, and daytime grogginess. Prolonged-release melatonin (Circadin®) addresses this by gradually releasing melatonin to mirror the body's physiological nocturnal secretion profile. By aligning with sleep promoting and physiological circadian mechanisms, it facilitates sleep onset, enhances the restorative quality of sleep, preserves critical sleep architecture (SWS and REM), and improves next-day alertness without compromising memory or physical stability and without risk of tolerance, dependence, and rebound insomnia.
Altogether, Circadin® is the result of a new way of thinking about sleep-wake disorders addressing circadian medicine in the management of insomnia with important impact on morning alertness. Long term efficacy and safety data support its use for insomnia in the 55 and older population.
