Abstract:
Cognitive behavioral therapy for insomnia(CBT-I) is the most prominent nonpharmacologic treatment for insomnia disorders. CBT-I is a multicomponent treatment package that usually includes stimulus control, sleep restriction relaxation training, and cognitive therapy and has emerged as the first-line chose of nonpharmacologic treatment for chronic insomnia.
The American College of Physicians (ACP) developed this guideline to present the evidence and provide clinical recommendations on the management of chronic insomnia disorder in adults. ACP recommends that all adult patients receive cognitive behavioral therapy for insomnia (CBT-I) as the initial treatment for chronic insomnia disorder. (Grade: strong recommendation, moderate-quality evidence)
Cognitive behavioral therapy for insomnia is efficacious for improving insomnia symptoms and sleep parameters for patients with comorbid insomnia. A small to medium positive effect was found across comorbid outcomes, with larger effects on psychiatric conditions compared with medical conditions.
